Upper and lower muscular folds of skin that move to cover and uncover the eyeball.

As you may be aware we are seeing a surge in cases in Colorado and Jefferson County. We are implementing some additional precautions in order to ensure we can remain open and continue to serve your eyecare needs.

IF YOU HAVE AN APPOINTMENT:

Please come in at the time of your appointment, ring bell at our front door, and we will conduct a health screening questionnaire and your temperature will be checked.

We are requesting ONLY the  patient with the appointment come in, unless patient is a minor or guardian/caretaker is required for the visit.

We are requiring that all patients over the age of 2 wear a mask that covers nose and mouth!

Please keep your mask on, and covering your nose AT ALL TIMES,  noncompliance will result in our staff requesting that you exit the building.

If you are a new patient, please fill out your paperwork ONLINE at BelleviewEyeAssociates.com, forms found under Patient Center.

IF YOU HAVE PRODUCT TO PICK UP:

Please pull up to the front door and call or text or office and we will run the product out to you
